The Annual Student Leadership Awards Program will be held on Tuesday, April 21 at 6:00 pm in the Astra Zeneca Auditorium, STC-145.
In preparation of this ceremony, Student Government and Student Life need your assistance in nominating students, faculty, staff and organizations for the following awards. After all nominations are received in the Student Affairs Office, a committee will be formed to select the winning nominees.
Nomination forms are available for download from this web site in Adobe Acrobat (PDF) format. The criteria for each award is available from this web site, click on criteria. If you are unable to print onto the form, please copy and past onto a word document and submit the word document. All forms must be typed.
