Student Leadership Awards
The Annual Student Leadership Awards Program will be held on Tuesday, April 21 at 6:00 pm in the Astra Zeneca Auditorium, STC-145.
Nominations
In preparation of this ceremony, Student Government and Student Life need your assistance in nominating students, faculty, staff and organizations for the following awards. After all nominations are received in the Student Affairs Office, a committee will be formed to select the winning nominees.
Nomination forms are available for download from this web site in Adobe Acrobat (PDF) format. The criteria for each award is available from this web site, click on criteria. If you are unable to print onto the form, please copy and past onto a word document and submit the word document. All forms must be typed.
Use one of the following forms to nominate for each specific award:
- Student Organization of the Year
- Martin Luther King Service Award
- Emerging Student Leader of the Year
- Persevering Leader of the Year
- Collaborative Leader or Organization of the Year
- Outstanding Cultural Program of the Year
- Outstanding Advisor of the Year
- Campus Community Development Program
When you have completed the form, please click the 'Submit by Email' button
at the top right of the PDF form. If you use webmail, you will have to email the
file that is generated to p.ohagan@usp.edu as an attachment.
Please note:
- A completed nomination form for these awards must be submitted to Patty O'Hagan in the Student Affairs Office, Room 118 or via e-mail p.ohagan@usp.edu by Monday, March 30.
- We encourage self-nomination for any category.
- Individuals may nominate in as many categories as they wish.
- Individuals may nominate more than one person/organization in the same category.
