Project Request Form

A Project Request is required when a service is needed beyond the scope of a regular work order. Before this form is filled out, please contact the Facilities Services department to evaluate what this project may involve. Some examples of what constitutes a project request are:

• Additional electricity needs for new equipment
• Office Renovations
• Heating and Air Conditioning improvements

All project requests are honored when scheduling and funding have been approved.
