Advisor of the Year Nomination Form

Purpose: To recognize and honor a faculty member of USP for excellence in academic advising. The nominee must be a faculty member of USP. The nominee can be your assigned academic advisor or another faculty member with whom you interact in an informal way for academic advising issues.

Procedure: Students, staff, and faculty may complete nominations. Advisors may nominate themselves. The Academic Advising Committee will review the nominations and the winner will be announced on June 12, 2008.

Completed forms can be submitted on-line by April 30, 2008.
